Nonprofit Partner Application Process

1 – Fill out the online application.

2 – You will then be prompted to download the Nonprofit Partner Application Packet. Fill it out and gather the following documentation.

  • A copy of your state and/or federal 501(c)3 tax-exempt designation letters

  • A copy of your most recent tax Form 990 or annual report (suggested)

  • Copies of state pharmaceutical handling licenses (if applicable)

3 – Send in a scanned copy of your Nonprofit Partnership Application Packet and the above documents to us via email, fax, or mail:
